What RACI actually means

R — Responsible
The people who do the work. There can be several.
A — Accountable
The single person who owns the outcome and signs it off. One name per row, always.
C — Consulted
People whose input is needed before the work can move. Two-way.
I — Informed
People told once it is done. One-way, no reply expected.

Four mistakes that kill a RACI chart

  1. Two names accountable. Each side assumes the other has it and nothing moves.
  2. Everyone consulted. If six people must weigh in, the task has no owner, it has a committee.
  3. Names instead of roles. People leave. Roles stay. Map the seat, not the person.
  4. No escalation path. A chart tells you who owns it, not what to do when they are on leave or asleep in another time zone.

Common questions

What is the difference between Responsible and Accountable?

Responsible people carry out the task. The Accountable person answers for whether it happened, on time and correctly. On a small team the same person often holds both.

How many tasks should a RACI cover?

Start with the ten pieces of work that get asked about most — approvals, handoffs, escalations, onboarding. A matrix with sixty rows never gets read.

Does this work for a company across several time zones?

A static chart does not, because it cannot say who is awake. That is where a live instance helps: coverage hours are part of your setup, so an overnight question routes to the team that is actually on shift.
